Reaching Home Victoria, also known Pathways to Home and Stability, supports individuals and families in Delta who are experiencing homelessness or are at risk of homelessness. The program focuses on housing stability, homelessness prevention, diversion from homelessness, and long-term support so that participants can move toward safety, stability, and independence.
The program serves people in Victoria who are unhoused, precariously housed, or at imminent risk of housing loss. EFry’s role within the program prioritizes single parents and children, while also supporting coordinated access to services for individuals and families with diverse needs. Participants may include people fleeing violence, people exiting institutions such as correctional facilities, hospitals, shelters, or treatment programs, people living with poverty or disability, and those who require additional support to secure and maintain safe housing.
Reaching Home Victoria provides housing-focused support through outreach, case management, housing navigation, income stabilization, prevention, and diversion services. Staff work with participants to identify their immediate needs, develop housing plans, access income assistance or disability benefits, connect with legal and systems-navigation supports, and build long-term stability. The program may also support participants with short-term practical assistance where appropriate, including helping address barriers that could lead to eviction, housing loss, or a return to homelessness.
The goal of Reaching Home Victoria is to reduce homelessness in Victoria by helping people access and maintain safe, stable housing. Through trauma-informed, gender-responsive, culturally safe, and client-centred support, the program works to prevent people from entering homelessness, help those who are unhoused move into housing as quickly as possible, and support participants to remain housed over time.
